OMMA Card Renewal Checklist

Renew on time and avoid a lapse. Start about a month before your card expires.
- Check your card’s expiration date — start the renewal about 30 days early.
- Confirm whether your license is 2-year (standard) and when it expires.
- Get an updated physician recommendation if your current one has expired.
- Have a clear digital photo of yourself ready (passport-style).
- Have proof of Oklahoma residency ready (driver’s license or two accepted documents).
- Have your application fee ready ($100, or $20 with SoonerCare/Medicare).
- Apply through the official OMMA portal — avoid third-party “middleman” fees.
- Save your approval email/PDF and updated license to your phone.
- Don’t let your card lapse — you can’t legally purchase without a valid license.
- Set a reminder now for your next renewal.
